A 26 year old man has just returned from holiday in Spain. He has a 3 day history of a productive cough and increasing dyspnoea. He is usually fit and well with no regular medications but is allergic to penicillin.
On examination, he has left basal crepitations consistent with a Pneumonia. His observations demonstrate that his blood pressure is 105/55 mmHg, his respiratory rate is 32 breaths per minute, his pulse is 105 beats per minute and his temperature is 38.9°C. He appears to be confused. His urea is 7.5mmol/litre. Treatment options are being considered.
