A 74 year old man presents with a fall, on a background of progressive nausea, fatigue and confusion. On examination, he is alert and reports the mechanism of his fall, which does not match that provided by his son, who witnessed the fall. On examination, he appears ataxic and he has a lateral rectus palsy. He has a past medical history of alcohol dependency.
