You are the FY2 doctor on a cardiology ward and go to prescribe Warfarin for Mrs Edwards, a ward patient who is being discharged later today. You took the blood test earlier and realise there is no INR result as the laboratory stated that the sample was received in the incorrect bottle. The patient is newly commenced on Warfarin and was only able to go home today if their INR was stable. You know they will be angry at what has happened.
