A 56 year old woman presents with a 12 hour history of increased dyspnoea, with bilateral leg swelling. For the last 2 days she admits to having intermittent central chest pain.
On examination, there are bibasal fine crepitations in her chest, with a raised Jugular Venous Pressure. Her blood pressure is 118/76 mmHg, her heart rate is 132 beats per minutes, her respiratory rate is 32 breaths per minute and her oxygen saturations are 97% on air.
