Troy is a 13 year old boy who has been brought to see you by his mother. Since returning from a holiday in Portugal two days ago, he has had worsening right sided ear pain and pruritis. He has not had any ear discharge or hearing loss and is otherwise well in himself. Past medical history includes Eczema and he does not take any regular medications and is allergic to aminoglycosides.
On examination, Troy’s right ear canal is erythematous and tender to palpation with debris visible. The tympanic membrane is visible and intact and he has cervical lymphadenopathy. Left ear examination is unremarkable. Observations are normal.
