Mr Rogers is a 54 year old lorry driver who has presented with a rash which first developed four weeks ago. The rash is itchy (moreso at night) and located mainly on his hands and feet. He has never had anything like this before and is not aware of any possible triggers. Three weeks ago, his pharmacist advised treatment with Permethrin cream (this was tried twice with no improvement of his symptoms). Examination revealed erythematous papules in the web spaces of his 3rd and 4th fingers and all his toes symmetrically. On his right hand you also notice two linear burrows with black dots at the proximal ends.
