Frank is a 65 year old man who was recently admitted to hospital with an exacerbation of Heart Failure and the hospital team increased his Furosemide dose until his next Cardiology review in 4 weeks. The change to his regular Furosemide dose was communicated on his discharge summary but this was missed by the GP who saw this letter.
